What is Intrahepatic Cholestasis of Pregnancy (ICP)?

ICP is a group of liver disorders specific to pregnancy which interfere with the flow of bile. Bile is a substance produced by the cells of the liver to aid in digestion of fats. During normal liver function, the bile which is produced is transported out of the cells and into the bile duct by special pumps. During Intrahepatic Cholestasis of Pregnancy, the cells are unable to transport the bile out of the cells normally, which leads to bile acids building up in the blood. Elevated bile acids in the blood are associated with increased risk to the unborn baby. It is important to note that Intrahepatic Cholestasis of Pregnancy is not a single disorder, but a heterogeneous group of many different disorders which all lead to elevated bile acids. This means that the disorder presents very differently in different affected women. 80% of cases are diagnosed in the third trimester, about 10% in the second trimester, and about 10% in the first trimester, with documented cases as early as 8 weeks pregnant.

What are the symptoms of Intrahepatic Cholestasis of Pregnancy (ICP)?

  • Itching – The most commonly noticed symptom of ICP is itching which can be moderate to severe. Itching due to Intrahepatic Cholestasis of Pregnancy does not typically respond to antihistamines. The itching is usually not associated with a rash, but a rash can, in some cases, develop as a result of scratching. The severity and location of itching can vary greatly. The most common location of itching is hands and feet, but some women experience itching on the arms and legs, the scalp, or all-over itching. Some women with the disorder itch everywhere except the hands and feet. In the majority of cases, itching is the only symptom reported.
  • Dark urine
  • Pale Stool
  • Right upper quadrant pain
  • Fatigue/Malaise
  • Mild depression
  • Pre-term labor
  • Nausea/lack of appetite
  • Rarely jaundice

What causes Intrahepatic Cholestasis of Pregnancy (ICP)?

Intrahepatic Cholestasis of Pregnancy is caused by a combination of factors including:

  • Genetics – Research has identified several genetic mutations which are associated with ICP. Not all genes have been identified. Many of these mutations cause defects in the bile salt export protein (BSEP), which is responsible for moving bile out of liver cells. Intrahepatic Cholestasis of Pregnancy can be considered a genetic disorder, even when there is no family history of the disorder. Mothers, sisters, and daughters of women affected by Intrahepatic Cholestasis of Pregnancy are at higher risk of developing the disorder, though it is not guaranteed.
  • Hormones – The initiation of ICP during pregnancy is influenced by the high levels of the hormones estrogen and progesterone, which are responsible for maintaining the pregnancy. The high levels of hormones further interfere with the liver’s ability to transport bile acids.
  • Environmental Factor– Intrahepatic Cholestasis of Pregnancy recurs in future pregnancies in 60-90% of cases, which indicates that environmental factors also play a role. It is not entirely understood why ICP sometimes does not recur, but research has found evidence that seasonal influences as well as improved nutrition may play a role. More women are diagnosed with Intrahepatic Cholestasis of Pregnancy in winter months, and selenium deficiency has been linked to the disorder, though it is not known if this is a cause or effect of ICP.

What is the treatment for Intrahepatic Cholestasis of Pregnancy (ICP)?

  • Early Delivery – Active management of Intrahepatic Cholestasis of Pregnancy reduces the risks associated with the disorder to that of an uncomplicated pregnancy, but there is no cure for ICP except delivery. Active management includes, most importantly, the medicine Ursodeoxycholic Acid (also known as UDCA, Actigall, Ursodiol, or Ursofalk), and early delivery, typically by 36-37 weeks gestation. In some cases, where medication is not controlling bile acid levels, delivery may occur even earlier.
  • Medication (Medical Option ~ Natural Options listed below) – Ursodeoxycholic Acid is considered the frontline treatment for the disorder. It has been proven safe in meta-analysis for both mother and baby, and research has identified many ways in which it potentially can provide a safer environment and protect the baby against harmful effects of bile acids until delivery can occur. It is a naturally occurring substance in the body, and has been proven to reduce the total bile acids in the bloodstream of women affected by Intrahepatic Cholestasis of Pregnancy. Sometimes other treatments will be used in conjunction with Ursodeoxycholic Acid with the aim of reducing maternal symptoms.

What are the risks associated with Intrahepatic Cholestasis of Pregnancy (ICP)?

Intrahepatic Cholestasis of Pregnancy puts affected pregnancies at higher risk for several complications. With active management it is believed that the risk of stillbirth is similar to that of an uncomplicated pregnancy (less than 1%).

  • Pre-term labor/delivery
  • Fetal Distress
  • Meconium Passage
  • Respiratory distress syndrome (RDS)
  • Failure to establish breathing (sometimes called fetal asphyxia)
  • Maternal hemorrhage
  • Stillbirth (Intrauterine Fetal Demise/IUFD)

Natural Options for the Treatment of Cholestasis

The idea here is to maintain a balance within the body, whereby keeping cholestasis at bay for as long as possible. Supporting the liver through dietary intake, supplements and herbs, keeping the bile acid levels lower for a greater duration of time.

As with anything I recommend to my clients, this is meant as a foundational guide to get you off and running in the right direction, with lots of options. Please do your own research from this foundation. Trust your body and your instincts, go with what feels right to you and listen to how your body responds.

Also, remember that any protocol works best when providing full support to the body. We tend to support one or two areas, leaving other areas of the body to make up the slack, whereby creating an unintended imbalance. The recommendations listed below are meant to be used for complete support for all areas in need during this time. Daily consistency and diligence are necessary.

Always discuss your options with your care provider.

Diet
-or-
     – Ensuring low fat intake, with complete removal of all saturated fats.
     – Ensuring that minimal fat intake is from healthy forms of fat. You may need to adjust some of the brewer’s diet recommendations to extremely cut fat intake. Focus on Fish, fruit and vegetables.
     – Aim for 8 – 10 Liters of water/day. Think of flushing the toxins out.
     – Foods high in Vitamin K, especially K2
     – Natto – Fermented Beans – Very high in K2
    – Get rid of all processed foods; digesting them makes your liver work harder.
– Foods that especially support the liver are apples, blackberries, dark grapes, plums raspberries, beets, asparagus, carrots, celery, leafy greens, onions, tomatoes and watercress. (some of those combined would be a great smoothie!)
Supplements
1) Vitamin K2, Jarrow Brand – This brand is specifically derived from Natto.
     – If you feel you need to supplement from your diet.
     – Acid levels need to be in balance for proper vitamin K absorption
     – Again, if you feel you need to supplement from your diet.
     – Selenium deficiency is one of the leading triggers for an increase in estrogen production, inducing bile flow impairment.
     – Lecithin is a type of unsaturated fat that is also referred to as a phospholipid. Your body produces some lecithin naturally, storing it in the bile in your liver to help metabolize fats.
     – It works as an internal lubricant within the body (also great while breastfeeding – prevention and treatment of blocked milk ducts and mastitis.)
4) SAM-e
      – Vitacost & Thrive Market are great places to purchase discounted, quality supplements!
     – S-Adenosyl-L-Methionine (SAMe) is a chemical that’s found naturally in your body that plays a role in liver and brain function. It’s also sold as a dietary supplement to treat a variety of conditions. According to the National Center for Complementary and Integrated Health, there is supporting research to suggest that SAMe is a safe and effective treatment for cholestasis.
     – You want 800mg per day. It is commonly sold as 400mg, so you would need double the dose.
      – Again sold at many natural food markets as well as Vitacost & Thrive Market, online.
      – This supplement naturally strengthens the liver’s structure and function, improves the body’s ability to filter toxins, drain acidic waste and decrease ammonia, whereby opening up the detox pathways in the liver and supporting kidney function.
Herbs
     – Nutritionally supports the liver’s ability to maintain normal liver function. It has shown positive effects in treating nearly every known form of liver disease. Milk thistle works due to its ability to inhibit the factors responsible for liver damage, coupled with the fact it stimulates production of new liver cells to replace old damaged ones.

     – Liver, spleen and pancreatic tonic, kidney tonic, helps dissolve kidney and bladder stones, stimulates liver, good for hepatitis, jaundice, anemia, acne and skin conditions. Fresh dandelion greens are highly nutritious and the flowers are edible, too.

Some of the herbs studies also talk about the benefits of artichoke herb, in combination with milk thistle and dandelion, for liver health and function. However, artichoke should not be used, when a blockage in the liver exists, which is what happens during cholestasis.

     – Liver Tincture – Liver Glow
     – Persephone’s Tea This counts as water intake! 😉
          – Steeping this in the correct manner is of the utmost importance.
               – Steep 1oz in 1 quart boiling water
               – Let sit for 2 hours
               – Drink 2 – 4 cups daily

2 Cups Goat’s Milk (preferably raw)

2 Cups Filtered water

1/4 Cup Goat Whey

1-2 tsp. Molasses

2 tsp. Grade B Maple Syrup

1/4 tsp. Probiotic (such as Raw Kids)

1 ml Cod Liver Oil (such as, Nordic Naturals baby DHA)

1 tsp. Unrefined Sunflower Oil

1 tsp. Olive Oil

2 tsp. Unrefined Coconut Oil

2 tsp. Nutritional Yeast Flakes

1/4 tsp. Acerola Powder

The body needs great nourishment during the healing and recovery stage of postpartum. In this time of adjustment and transition it can often be difficult to find the time necessary to eat adequately for your health, energy and the support of a well established milk supply for your new baby. This salsa makes that easy! Packed with omegas, healthy fat, a ton of nutrients, and added protein depending on your dipping choice.

This recipe was passed down to me from one of my Midwives, and has been a blessing throughout my breastfeeding journeys.

Ingredients

Use all organic ingredients if possible.

Salsa Ingredients

1 Large Tomato Diced

1 Can Organic Sweet Corn Kernels

1 Can Organic Black Beans

1 Bunch Green Onions Diced

1 Bunch Cilantro Finely Chopped

1 Red Bell Pepper Diced

1 Large Avocado (Diced – to be added with the drizzle ingredients)

5 – 8 Cloves Garlic Finely Chopped

Drizzle Ingredients

1/3 Cup Freshly Squeezed Lime Juice

1/3 Cup Flax Seed Oil

1/4 Cup Olive Oil

1 Drop Lime Essential Oil (optional)

1 Drop Fennel Essential Oil (optional)

Seasonings

Sea Salt to taste

1/4 tsp. Ground Cayenne Pepper (Optional) (May add more, if you like spicy!)

Mix all salsa ingredient together and stir.

Pour drizzle ingredients over the top and stir. Add avocado. Avocado may brown due to air exposure, but will still be good in the lime juices.

Add seasonings and stir again.

Stores in fridge for about a week depending on your fridges internal temperature.

Preparing for childbirth is much like preparing for a voyage around the world by sea.

If you have never taken a voyage by sea, and have no experience sailing, it would be nearly impossible to set out without an experienced guide and know that you are going to have a smooth voyage from beginning to end, that you are going to arrive at you intended destination point having met all of your goals for the planned journey.

A doula is your guide to understanding the birth process, knowing where you are in each moment, what to expect around the next corner, how to handle each wave as it comes crashing your way, how to be supportive, how to meet the changing needs of the laboring mother as the weather out to sea changes and intensifies.

A doula is the voyage guide … the lighthouse on the shore … the whisper through the winds of change

Mom + Baby + Dad = “The Birth Team”

A Doula = “The Nurturer of the Birthing Process”

Giving “The Birth Team” the physical, emotional and informational support needed along the way.

A Doula is not only support for the birthing mother, she is also support for the birthing father.

This is a journey that mom and dad are taking together with their baby.

A Doula’s role is to facilitate that journey and protect the bond between “The Birth Team” as they work together.

A Doula is not a replacement for a dad’s role in the birth process. Quite the opposite in fact.

  • A Doula is an extra pair of hands in many laboring positions which require more than one support person attending to mom
  • A Doula is an extra pair of ears in recognizing the changes in the progression of labor and keeping dad informed along the way
  • A Doula is an extra pair of eyes in assuring proper advocacy of the desires and wishes of your birth plan, so dad can be completely focused on mom
  • A Doula speaks the language of birth and keeps mom and dad well informed about the stages of progression and the changes in care that mom and baby will need throughout each stage
  • A reduction in the amount of stress and confusion which can be caused by an often intimidating hospital room and staff who are not always available to answer immediate questions
  • The Partner doesn’t have to worry about remembering everything learned in the childbirth class because the Doula is there to provide guidance, reassurance and provide information on the ‘normalcy’ of the birth process
  • With a Doula present, dad will have his emotional needs met (ie. stresses, worries, concerns, etc) so dad can remain positively focused on mom and his baby and his role in “The Birth Team”
  • Having a Doula means dad can step away for a spell to use the restroom, have a light snack, take a brief power nap, or just catch his breath and have a moment to reflect, with peace of mind that mom and baby are being well supported in his absence
  • Doula’s provide comfort to dad’s and help them remain calm and collected during a time that might otherwise have felt stressful
  • Doula’s provide heavy doses of guidance & encouragement keeping “The Birth Team” feeling confident in themselves and the birth process
  • A Doula can help you and your partner have a more intimate birth experience by helping to set the tone in a non-intimate setting, such as in a cold hospital room, and help to protect the intimacy desired in a hospital birth

Skin Sensitivities ~ Eczema ~ Chemicals ~ Toxins ~ Carcinogens ~ Cost ~ Cloth Diapers

The above are just a few of the reasons I chose to make laundry soap for my family, rather than purchasing laundry detergent from the store. I have been making laundry soap for my family of seven for the last 3.5 years and I honestly cannot imagine how we used such awful products for so many years!

I have sensitive skin myself… Having a growing family I am always concerned about what I am exposing my babies too, and being mindful about properly caring for their very sensitive and new skin. Preventing skin allergens or eczema outbreaks was important to me.

Traditional detergents contains a rather scary “laundry list” of nasty ingredients. Many of which are toxic or carcinogenic to our bodies. They may smell “pretty,” but that smell comes from synthetic, artificial fragrances. I wanted to naturally scent my soap using my essential oils!

For a family of seven, we spent a GREAT deal of money (monthly / yearly) on detergent. Like most families, anyplace we could pinch pennies was something we were interested in pursuing. Cutting out this expense, almost completely, has been an amazing blessing to our family!

We also cloth diaper. So prior to making my own laundry soap, we were purchasing a detergent for our clothing AND a detergent for cloth diapers. Making my own soap, means this is no longer necessary. The chemicals found in almost all store bought detergents are not safe to use on cloth diapers. It can actually damage them, permanently!

My Tried & True Recipe

(This recipe makes enough soap to last 2.5 – 3 months for my family of seven)

This is safe for HE machines, I have used it in mine for the past 3 years.

It is also important to note, that with all natural soaps, there are no suds during the washing process!

 

Step 1) Ingredients you will need:

1 Cup – 20 Mule Team Borax (detergent booster)

1 Cup – Super Washing Soda (not baking soda)

2 Bars – Dr. Bonner’s Castille Bar Soap (any scent you would like… Lavender, Citrus Orange, baby un-scented).

You can also use Fels-Naptha ( This is a stronger soap base and is great on work clothes, but can be a bit harsh on baby skin.

 

Essential Oils for Cleansing, Purifying, and Scenting (Some of my favs – Orange, Lavender, Purify, OnGuard, and my husband likes Sandalwood)

 

 

Step 2) Kitchen Tools you will need:

Large Cooking Pot (non-Teflon)

A Long Handled Wooden Spoon & a Ladle (not pictured)

A Large Funnel

A Box Grater

A Glass Bowl

6 Containers (to store & use your soap) ( I use old orange juice containers)

You want about six containers for this recipe.

My laundry soap yields about 530 fl. oz.

 

 

 

 

 

Step 3) Mark containers and pre-fill with water:

To make the process quicker for me, I mark all of my containers with the starting amount of water.

 

This is about 45 – 50 fl. oz.

Here I have all 6 bottles.

 

Each filled with about 45 – 50 fl. oz.

 

 

 

Step 4) Personal Touch:

I then add the scent I’d like for this batch.

 

I tend to change it often, depending on season. On Guard during the Holidays, floral in the spring, etc.

 

We can do different scents in each bottle, to customize them to specific family members.

 

My husband loves sandalwood, I love lavender, and really like serenity for my children.

Essential Oils have been added to each bottle

 

 

 

 

Step 5) Grate Soap:

Fill your large stock pot with about 90 fl. oz. of water.

Put on the burner over med-high heat.

While the water is warming, grate your soap, just as if it were a block of cheese. =)

This quickens the melting process of the soap.

 

 

Step 6) Melt Soap:

Add grated soap to the pot.

Stir frequently as it melts.

Do not allow your pot to come to a full boil.

 

 

 

Step 7) Super Washing Soda:

Measure out 2 cups of Super Washing Soda.

Add to the melted soap water.

 

Stir it in so the soda powder is not sitting on the bottom of the pot.

 

 

 

Step 8) Borax:

Measure out 2 cups of Borax.

Add Borax to water, soap and soda mixture.

 

Again, stirring to mix well.

 

 

 

 

Step 9) Thicken:

Water will become very sudsy.

Stay nearby, stirring often, and controlling the temperature.

You want your pot to come just close to a boil, without actually coming to a boil.

Should it get to hot, or sit too long without being stirred, the mixture will rise right up out of your pot, and all over your stove!

This can happen VERY quickly! Let’s not ask me how I know this… 😉

 

 

Step 10) Transfer:

Once the laundry soap mixture has began to thicken up ( usually about 10 minutes) ( think brown gravy consistency), you can remove pot from heat.

Ladle soap mixture into pre-filled containers.

 

Ensure the funnel is aimed into the water, and not onto the sides of your container

 

The cool water in the container helps to off-set the temperature from hot mixture, and will cause it to thicken more (think, making pudding).

When filling your containers with soap, you want to leave enough empty space in each, to allow room for coagulation to occur.

 

 

 

 

Step 11) Cool:

Allow containers to sit for about 30 minutes with the caps off, to cool, before covering and storing for the night.

 

 

 

 

 

Step 12) Store Overnight:

As the mixture sits overnight, it will become very very thick!

In the morning, soap containers will have coagulated.

 

 

 

 

 

Step 13) Shake & Prep for Use:

You can shake the soap ( or stir with the handle of a wooden spoon).

 

Then top off the container with cool tap water, and shake again.

 

I tend to shake the container before each use.

 

I use 1/2 – 3/4 cup per load, depending on what I am washing.

Understanding the Difference Between a Doula and a Midwife

“I didn’t know there was a difference between a Doula & a Midwife”

“I thought when I hired my Midwife, she would also be my Doula”

“My Midwife said, I don’t really need a Doula, because she will be there”

“I didn’t realize how much work my Midwife wanted me to do on my own before she will be present”

I often am asked about the difference between a midwife and a doula. It seems, that many are under the impression that you need to either choose a midwife or a doula, or that you do not need a doula if you have a midwife. However, a midwife and doula have distinctly separate roles when it comes to the kind of support and care you receive during your pregnancy, your birth, and during the postpartum period.

Let’s take a glance at some of the specific roles of a Doula & a Midwife during your very special journey…

Your Midwife: Personalized Maternity Care

  • Oversees the medical safety of your pregnancy and birth
  • Runs prenatal tests
  • Advises you on health & nutrition during pregnancy, birth and postpartum
  • Monitors you and baby during labor and birth
  • Clinical tasks such as, temp, pulse, BP, and FHR
  • Performs physical examinations prenatally, during labor and postpartum
  • Consults with an obstetrician if a medical complication arises which is out of the midwifery scope of practice

Doulas, on the other hand, are not medical professionals. Doulas provide physical, emotional & informational support during pregnancy, birth, and the immediate postpartum period.

Your Doula: Physical, Emotional, & Information Support

  • Establishes a prenatal relationship with you
  • Helps you and your partner articulate your ideal vision for your birth
  • Directs you to outside resources within your community for pregnant women and families
  • Ensures you and your partner feel calm, informed and supported during labor
  • Guides your partner in engaging and supporting during the labor and birth process
  • Uses natural tools and hands-on techniques to help you manage and cope with the intense physical sensations of labor and birth
  • Makes suggestions regarding laboring and birthing positions depending on how your labor is progressing
  • Makes sure you understand all of your options so you can make informed decisions every step of the way
  • Ensures you feel confident communicating your needs to your health care provider
  • Provides postpartum emotional support and help with breastfeeding initiation

While doulas do not provide medical care, research shows they do increase your likelihood of avoiding unnecessary medical interventions, greatly decrease your chance of a cesarean birth, and significantly decrease a woman’s need for analgesia, compared to usual care.

Do I need BOTH a Midwife and a Doula?

Doula support is an amazing complement to the care you will receive from your midwife. It is important to understand that the ultimate responsibility of your Midwife is the health and safety of you and your baby. Your midwife needs to be present, alert and at the top of their game during the active stage of your labor, pushing and the birth of your baby.  If your Midwife were to provide 12+ hours of physical and emotional labor support, they are going to be tired, spent, and not have the level of alertness necessary should their be a medical emergency at any point of your labor and birth process.

Most Midwives want you to labor on your own as long as possible, and they only come (or check you into the birth center), once labor has been well established and you are nearing the home stretch. They call it the 511. When contractions have reached 5 minutes apart, lasting 1 minute or more in length, for 1 consistent hour. For many women, this can mean laboring for 12, 24, 36 hours on their own.

They will check vitals, asses Mother and Baby for the medical safety and progression of the labor process, and then hang back, and let you labor on your own. They will occasionally provide physical and verbal support, but it is not given in a manner that will hinder their role as the medical care provider of you and your baby.

Your doula, however, comes to you early in your labor, when you first begin to feel that you are in need of support and guidance, and she will stay with you until after the birth of your baby. Doulas love being part of the early labor stage and following your journey to the end. Their continual support can make such a difference on the overall experience of the birth process for both you and your partner!

When you seek out a Midwife, it is because you want to have a natural, un-medicated, un-intervened upon childbirth, with the freedom to birth your way, on your terms. Using both a midwife and a doula will give you the full spectrum of support and medical care you need, to achieve the birth you want, and ensure that you are able to naturally cope every step of the way.

Together, having BOTH a midwife AND a doula, means you will be supported and nurtured in every way as you birth your baby. Your partner, your midwife, your doula … They create your birth team, your village…

Midwives and doulas work hand-in-hand together, with a great respect for each other’s roles in supporting the birth you desire. However, they are very different professions, with different roles and responsibilities to you, as the birthing family.
